There are a number of lifts. You can pull up right outside in a cab. There are a few steps up to reception but they have concierge who will help you. They are super responsive via email so email ahead with any needs.

I'm sorry I can't answer your question, but if it's too late to change your hotel, I think you will find plenty of restaurants nearby that have outdoor seating and wonderful food (such as Da Enzo 29 just across the river and Vecchia Roma in the Jewish Ghetto, just a couple of blocks away). Just use the restaurant filters in Trip Advisor to select the neighborhood (Ghetto or Trastavere), and select outdoor seating. We found great restaurants that way. The concierge at the hotel is wonderful, and will help you with restaurant reservations, which are definitely needed for Da Enzo 29 (we made ours well in advance on the Trip Advisor website). I would definitely recommend Vecchia Roma for a romantic evening - the food is terrific and so is the service.
We ate our first dinner at the hotel's rooftop restaurant, and it was good, but nothing special. There are just a few tables, and it can be windy. The garden area is pretty, but you can eat your breakfast just as easily indoors and look at the garden.
